Aangezien ik wel pssm getest heb ga ik volgend jaar de 5 panel test ook nog laten doen nu in de dure maanden is dat niet gewenst

Erg leuk, kreeg van een medebokker 2 pbtjes, ze had de hele pssmsite doorgelezen en zo al 5 schrijffoutjes eruit gevist. En een aanvulling op de symtpomenlijst van iemand anders. Hopelijk blijft dat zo gaan! Dan wordt de site alleen maar beter en beter en vollediger. :j

Heb toevallig van de week je site bekeken. Inhoudelijk prima!

Ik mis wel wat illustraties en met name ook filmpjes. Op Youtube staat een aantal filmpjes van paarden met een pssm aanval en paarden die chronisch spierkramp(jes) hebben door pssm. Zeker als je er net mee geconfronteerd wordt is dat beeldmateriaal heel handig, je hebt anders nog geen idee hoe het er uitziet.
Ikzelf heb ze ook pas veel later gezien (nadat Buzzy haar eerste grote aanval had gehad) en ik zou willen dat ik het eerder had gezien.

En ik stuur je nog ff een pb!

hey! goed initiatief! suggestie: er zijn een heleboel symptomen naast tie-up: moeite met lengtebuiging,kribbig met opzadelen/opstappen,borstelen,depressiefslikproblemen,slokdarmverstopping,ronkende ademhaling tijdens racen in de wei bijvoorbeeld. jeuk door de spierspanning,overdreven staartzwiepen in rust maar ook tijdens het rijden,onverklaarbare agressie,bokken,steigeren,bijten in de eigen benen,defensief gedrag zonder echte aanleiding....veel pssm-ers worden koppig/vals en onbetrouwbaar genoemt,tot de diagnose er is.

Wat fijn dat dit topic er is! Via een topic dat ik aanmaakte bij Medisch, kwam ik hier terecht. Echt bizar dat een aantal zaken die ik lees, precies de soms 'vage' symptomen van mijn (weliswaar KWPN) paard beschrijven (de DNA test is vorige week verstuurd). Zoals spierbevangenheid na een rhino enting (die ik maar liefst 3x heb gegeven...). Hoe zijn jullie ervaringen op de lange termijn qua intensiteit van PSSM? Kan het erger worden of blijft een 'mildere' vorm/minder gevoelig paard of 'heftige' vorm/gevoelig paard min of meer gelijk?

sphinxie, het verloop is heel wisselend, de mijne (moeder en zoon) krijg ik al maanden niet goed. andere pssm-ers doen het dan weer stukken beter, gelukkig. DNA test heeft weinig zin bij een KWPN-er, die hebben,als ze positief zijn, 9 van de 10x pssm2, dit kun je niet via DNA testen, enkel via een spierbiopt. als je FB hebt moet je anders eens op pssm-forum zoeken, lid worden en dan kun je echt bijna alles vinden over de typen etc.

Het Facebook forum is idd echt een aanrader. Over PSSM2 is helaas nog weinig bekend in hoe het verschilt van PSSM1. Qua verloop weet ik dus ook niet wat de verschillen zijn. Bij mijn PSSM1 positieve merrie heb ik iig gemerkt dat het met het verstrijken van de jaren erger wordt; dat ze steeds makkelijker symptomen heeft.

Dit heb ik uit een artikel gecopieerd, waarvan ik de link kwijt ben 8)7 , maar het omschrijft het verschil tussen type 1 en 2. Het artikel is overigens geschreven door de toonaangevende professor op dit gebied, Prof Valberg:

Type 1 PSSM

Prevalence:
The GYS1 mutation responsible for type 1 PSSM is present in over 20
different horse breeds (MCCue et al 2010, Baird et al 2010). It is estimated to have
emerged as far back as 1600 yrs ago when the great horse was being developed from
European draft and light horse breeds to carry knights with heavy armor into battle8. The
best estimates of disease prevalence come from random sampling of horses of a variety of
breeds, however, when breeds are rare nonrandom sampling may be necessary. The
highest prevalence of PSSM appears to occur in draft horses derived from Continental
European breeds (Baird et l 2010) (Table 1). In fact, many Continental European drafts
are homozygous for this dominant trait (90% prevalence of PSSM in Trekpaards with
40% of tested Belgian Trekpaards homozygous). In contrast, the prevalence of PSSM is
very low prevalence in United Kingdom-derived breeds such as Shires and Clydesdales
(McCue et al 2010).

Prevalence estimates of type 1 PSSM in Quarter Horses range from 6 to 10% of the breed
being affected, and 6 to 8% for American Paint and Appaloosa horses11, 12. The highest
frequency of Type 1 PSSM occurs in halter Quarter Horses (28% affected) and the lowest
frequency in racing Quarter Horses12. The GYS1 mutation has been identified in
approximately 72% of Quarter Horses diagnosed with PSSM by muscle biopsy and in
18% of Warmbloods of a variety of types diagnosed with PSSM by muscle biopsy9. The
prevalence of type 1 PSSM is very low in light horse breeds such as Arabians and
Thoroughbreds (Table 1).

Etiology:
Glycogen is formed by the enzymes glycogen synthase which provides straight
α 1→4 glycosidic linkages and branching enzyme which adds branched α 1→6
glycosidic linkages to the glycogen molecule. The autosomal dominant GYS1 mutation
produces a gene product, glycogen synthase, which has an arginine substitution for
histidine at codon 3098. The effect of this amino acid substitution is a higher than normal
activity of glycogen synthase both at rest and when activated by glucose 6-phosphate. As
a result, skeletal muscle of PSSM horses has 1.5 to 4 fold higher concentrations of
glycogen than normal horse muscle4. A less branched abnormal polysaccharide
eventually accumulates in PSSM muscle by 18 months of age13. The abnormal
polysaccharide appears to be the result of a much higher ratio of glycogen synthase to
branching enzyme activity in PSSM muscle resulting in accumulation of a less branched
abnormal glycogen molecule. The accumulation of abnormal polysaccharide is in itself
not the cause of muscle dysfunction in PSSM since foals as young as 1 month of age may
show evidence of muscle damage prior to the formation of abnormal polysaccharide in
skeletal muscle fibers. Rather, the persistent glycogen synthase activity in type 1 PSSM
horse muscle appears to disturb the normal flux of muscle energy metabolism during
exercise.

A deficit in cellular energy within individual muscle fibers of type 1 PSSM horses has
been found after only 6 to 15 min of exercise, and is manifested by an increase in the
purine nucleotide inosine monophosphate (IMP)14. A diet high in nonstructural
carbohydrates (NSC) further exacerbates clinical signs of PSSM15. This may potentially
be the result of elevation of serum insulin by a high NSC meal which serves to activate
the abnormally regulated glycogen synthase enzyme activity further exacerbate
regulation of energy flux in skeletal muscle with the GYS1 defect.
The precise link between diet, enhanced glycogen synthase activity, and muscle cell
damage has not been fully elucidated. Glycogen synthase activation is reciprocally linked
to inactivation of myophosphorylase, the enzyme necessary for release of glucose (G6P)
from glycogen. Constitutively active glycogen synthase enzyme further stimulated by
insulin could impair release of glucose for energy metabolism by inactivating
myophosphorylase. In addition, high grain diets favor fatty acid synthesis within the
muscle cell as a result of increased muscle citrate concentrations 16. If fatty acids within
myofibers are directed toward synthesis rather than metabolism this could further
decrease substrates for energy metabolism in the muscle cell. During the first 20 min of
exercise muscles are dependent on glycogen for energy and if the mutation disrupts
glycogen availability and impairs the availability of fatty acids exercise intolerance may
arise.

Acute clinical signs:
Horses usually show signs of PSSM at on average 6 years of age,
however, this can range from 1 to 14 years of age. Some horses with type 1 PSSM are
asymptomatic. In general, owners describe horses with type 1 PSSM as having a calm
and sedate demeanor. Acute clinical signs include tucking up of the abdomen,
fasciculations in the flank, muscle stiffness, sweating reluctance to move forward and
overt muscle contractures. The hindquarters are frequently most affected, but back
muscles, abdomen, and forelimb muscles may also be involved. Signs of pain can last for
more than 2 hours and about 10% of cases becoming recumbent. Muscle pain often
occurs with less than 20 minutes of exercise at a walk and trot especially when horses are
unfit at the commencement of training or after horses have had a substantial period of
rest. A diet high in NSC exacerbates theses signs. During an acute episode of ER, horses
with type 1 PSSM often have markedly elevated serum CK activity of >35,000 U/L and
myoglobinuria may be present. Severe colic-like pain post-exercise and myoglobinuric
renal failure are less common presenting complaints. Some owners report a seasonal
incidence to development of acute clinical signs which some have attributed to quality of
grass available at the time.

Chronic clinical signs:
Light breeds: Chronic signs of type 1 PSSM in riding horses include a lack of energy
when under saddle, reluctance to move forward, stopping and stretching out as if to
urinate and a sour attitude towards exercise. Horses may have a combination of low grade
reluctance to exercise, poor performance and repeated episodes of ER. The range of
severity of clinical signs of PSSM can be wide with some horses being asymptomatic and
others completely incapacitated. Serum CK activities are often elevated in untreated
Quarter Horses, even when horses are rested. While horses are symptomatic, CK will
usually increase by 1000 U/L or more 4 hours after 15 minutes of exercise at a trot. The
median CK and AST activity for all PSSM Quarter Horses with muscle biopsies
submitted to the University of Minnesota was 2,809 and 1,792 U/L, respectively.
Affected Quarter and Paint Horse foals and weanlings may develop rhabdomyolysis
without exercise.

A small number of Quarter Horses and Paint horses have both the GYS1 mutation and a
genetic mutation for malignant hyperthermia (RYR1) which results in particularly severe
signs of ER and a limited response to diet and exercise changes17. During an episode of
ER, excessively high body temperatures may develop and sudden death can occur in
horses with the RYR1 mutation.

Draft Horse and Draft Crosses:
The average age of draft horses diagnosed with PSSM is
about eight years of age18. Many draft horses with PSSM are asymptomatic. Signs of
severe rhabdomyolysis and myoglobinuria may occur in horses fed high grain diets,
exercised irregularly with little turn out or horses that undergo general anesthesia19. Other
signs of PSSM in draft horses include progressive weakness and muscle loss resulting in
difficulty rising in horses with normal serum CK activity. Pronounced weakness is more
prevalent in homozygotes for the GYS1 mutation. Gait abnormalities, such as excessive
limb flexion, fasciculations, and trembling are also reported in draft horses. Although the
condition Shivers was previously attributed to PSSM, a recent study found no causal
association between these two conditions18. The median serum CK and AST activities in
draft horses from which biopsies were sent to the University of Minnesota was 459 and
537 U/L, respectively.

Diagnosis:
A genetic test for the GYS1 mutation can be performed on whole blood or
hair root samples in North America at the University of Minnesota
(http://www.vdl.umn.edu/vdl/ourservices/ ... cular.html) and in Europe by
Laboklin (http://www.laboklin.co.uk/laboklin/GeneticDiseases.jsp). MH testing is
provided at the University of Minnesota and the University of California, Davis.
Muscle biopsy can also provide a means to diagnose type 1 PSSM. The distinctive
features of type 1 PSSM in muscle biopsy samples are numerous subsarcolemmal
vacuoles and dense, crystalline periodic acid Schiff’s (PAS) positive, amylase resistant
inclusions in fast twitch fibers10. Genetic testing provides the gold standard for diagnosis
because a false negative diagnosis of type 1 PSSM by muscle biopsy may occur if biopsy
samples are small or if horses are less than 1 year of age.

Type 2 PSSM

There is much less known about type 2 PSSM, because as it turns out, previous research
on PSSM has largely involved horses with type 1 PSSM. Current knowledge of type 2
PSSM is based on retrospective evaluation of cases diagnosed with PSSM by muscle
biopsy20 that are now known be free of the GYS1 mutation and a few years of prospective
clinical cases.

Prevalence:
Approximately 28% of cases of PSSM diagnosed by muscle biopsy in
Quarter Horses do not have the GYS1 mutation9. Type 2 PSSM seems to be more
common in higher performance horses such as barrel racing, reining and cutting horses
compared to the high prevalence of type 1 PSSM in halter horses. About 80% of cases of
PSSM diagnosed by biopsy in Warmbloods have type 2 PSSM. Breeds affected include
Dutch Warmbloods, Swedish Warmbloods, Hanoverians, Friesians, Selle Francais,
Westfalian, Canadian Warmblood, Irish Sport Horse, Gerdlander, Hussien, and Icelandic
horses. Many other light breeds have also been diagnosed with type 2 PSSM including
Morgans, Standardbreds and Thoroughbreds. Type 2 PSSM also occurs in Arabians,
however, in my experience this breed is distinct in that it often has amylase-resistant
rather than amylase sensitive polysaccharide but is negative for the GYS1 mutation.

Etiology:
The cause of type 2 PSSM is currently unknown. It may well be that there are a
group of conditions that have separate etiologies but share common findings of glycogen
accumulation and poor performance. A heritable predisposition is suspected in Quarter
Horses but yet to be proven.

Acute Clinical signs:
Horses with type 2 PSSM do not necessarily have the same calm
temperament as horses with type 1 PSSM. In adults, acute clinical signs of
rhabdomyolysis are similar between type 1 and type 2 PSSM. Muscle atrophy after
rhabdomyolysis is a common complaint in Quarter Horses with type 2 PSSM. There are
more Quarter Horses less than one year of age reported with type 2 PSSM than type 1
PSSM and these foals may present with an inability to rise or a stiff hind limb gait.
Chronic clinical signs: Chronic signs of type 2 PSSM are often most closely related to
poor performance rather than recurrent ER and elevations in serum CK activity. An
undiagnosed gait abnormality, sore muscles and drop in energy level and willingness to
perform after 5 -10 min of exercise are common complaints in Quarter Horses with type
2 PSSM. Warmbloods with type 2 PSSM have painful firm back and hindquarter
muscles, reluctance to collect and engage the hindquarters, poor rounding over fences,
gait abnormalities, and slow onset of atrophy. The mean age of onset of clinical signs in
Warmbloods is between 8 and 11 years of age with the median CK and AST activity
being 323 and 331U/L, respectively.

Diagnosis:
Type 2 PSSM must be diagnosed by muscle biopsy where increased or
abnormal PAS positive material that is usually amylase-sensitive is apparent particularly
in subsarcolemmal locations. Determination of what constitutes an abnormal amount of
amylase-sensitive glycogen can be subjective. False positive diagnosis is possible for
type 2 PSSM in highly trained horses that normally have higher muscle glycogen
concentrations or in formalin fixed sections which show a greater deposition of
subsarcolemmal glycogen even in healthy horses. Other histopathological features that
may be present with both type 1 and type 2 PSSM include muscle necrosis, macrophage
infiltration of myofibers, regenerative fibers, and fiber atrophy. Some laboratories grade
polysaccharide accumulation as mild, moderate, and severe where mild accumulation
represents a category which has a higher chance of being a false positive diagnosis. Mild
PSSM cases in particular should receive a full physical examination to ensure that there
are not other underlying causes for performance problems.

A muscle biopsy of any locomotor muscle that provides a 2 cm by 1 cm block of tissue
for evaluation is often sufficient for diagnosis of PSSM. The site most easily sampled in
the field using an open surgical approach is the semimembranosus or semitendinosus
muscle. Clinics that can rapidly process muscle for frozen sections often use a modified
Bergstrom biopsy instrument inserted into the gluteal muscle through a 1 cm incision. A
diagnosis can be made irrespective of diet and proximity of sampling to recent episodes
of rhabdomyolysis. Preferences for fresh and formalin fixation vary between laboratories.
The University of Minnesota Neuromuscular Diagnostic Laboratory prefers fresh
samples as these a variety of histochemical stains can be performed to characterize
muscle diseases.

Nuttig stuk! Als ik dit zo lees neig ik veel meer naar PSSM 1 dan naar PSSM 2 bij mijn merrie, wat apart is omdat ze KWPN'er (of eigenlijk ook half Oldenburger) is.

Hebben jullie ervaring met het rijden met een hartslagmeter ter voorkoming van een aanval van spierbevangenheid of hier theorieën over? Omdat een verhoogde hartslag voorkomt bij spierbevangenheid ben ik na de 2e keer regelmatiger met een hartslagmeter gaan rijden (ik deed het al af en toe ihkv meten is weten). Voor mijn gevoel ben ik daarmee 1x ernstige(re) acute spierbevangenheid voor geweest (haar hartslag bleef bij een rondje stap tussendoor in de training, een stuk hoger dan haar normale hartslag in stap en dat was voor mij reden om dan gewoon te stoppen i.p.v. verder trainen). Ik heb toen een paar dagen rustig aangedaan en toen weer opgebouwd. Ik heb 'r daarna ook nog een keer spierbevangen gehad ondanks de hartslagmeter, maar toch...

Nu ga ik denk ik ik iets heel doms vragen :D maar zit er in de Oldenburger ook koudbloed gefokt? Koudbloeden staan nl wel bekend om hun hoge PSSM1 factor. De Quarter, Paint en Appaloosa hebben hun PSSM1 daar hoogstwaarschijnlijk ook vandaan.

Ik denk dat een verhoogde hartslag al een reactie is op het spierbevangen zijn/aan het worden zijn. Misschien kun je wel ergere schade voorkomen als je het merkt en je dus kunt stoppen voordat er al te veel spierschade optreedt. Goed idee!

Voor zover ik weet niet :).

Denk idd dat het zo is als jij zegt, als de hartslag hoger wordt is het proces al begonnen maar wellicht kun je wel erger voorkomen.

misschien heel stom en ik lees het nu pas maar mijn paard is dus ook bijna spierbevangen( ik was er op tijd bij ) geraakt nadat ik hem geent had tegen rhino. ( ook heb ik een jaar getobt met erge mok ) is daar meer over bekent toevallig ? nu ik hem dus deze enting niet meer gegeven heb en hem op een normaal rantsoen heb heb ik nog meerder malen zijn bloed laten controleren en nooit meer afwijkende waardes gehad.
het is in mijn ogen erg toevallig dat ik nu dus na het niet enten van rhino niets geen klachten meer heb ook niet van mok en dergelijke
dus misschien zit er ook een link richting de rhino enting?

Ik heb papieren ingestuurd naar de verzekering ivm het voornemen om ons PSSM paardje te laten inslapen.
Nu kreeg ik bericht dat er van schade-uitkering geen sprake kan zijn doordat de oorzaak voor de geboorte/ingang van de dekkingstermijn ligt.
Heeft iemand van jullie wel een schadevergoeding gekregen?

Ik ga het zelf ook nog wel even uitzoeken maar ik vind het een beetje raar.
Bij haar geboorte was ik nog helemaal niet bekend met pssm (2010) en bovendien was de merrie al verzekerd en heb ik het veulen gelijk aangemeld.

voelt altijd een beetje raar om over geld te praten terwijl je je paardje laat inslapen maar toch..

Nou ja, verzekeren doe je toch mede om dit soort situaties financieel mee op te vangen dus het lijkt me logisch dat je dit uitzoekt voordat je je paard laat gaan.

Staat er iets in de polis over genetische aandoeningen en wat de voorwaarden daaromtrent zijn als het paard wordt opgenomen in de verzekering? Wat nou als je niet had getest op PSSM omdat je simpelweg niet van het bestaan ervan af wist en het paard wordt ingeslapen omdat het chronisch spierbevangen dan wel kreupel is? Wordt het met die verklaring door de dierenarts wel geaccepteerd? Dat zou je misschien eens uit kunnen bekijken.

in elke polis staat dat het dier gezond moet zijn voor de verzekering wordt aangegaan. omdat pssm een genetische aandoening is was het dier dus niet 'gezond' en heeft de verzekering dus grond om niet uit te keren. treurig maar helaas ;(
des te meer reden om elk paard te laten testen

@slootje: ik weet niet hoe je da evt zou willen meewerken door bijv te zeggen dat het paard een hartstilstand oid heeft gehad....
